It is very important to have a primary care provider(PCP). When you are established with a PCP they become familiar with your personal medical history and are able to provide better management of acute and chronic conditions. Having a PCP leads to better management of chronic conditions, lower overall healthcare costs, and a decrease in hospitalizations, urgent care and ER visits.
Your primary care provider can identify health issues in their early stages before they become major problems. They offer preventive care such as immunizations, regular imaging & lab testing, such as cholesterol, blood pressure, flu shots and cancer screenings, as well as diet and lifestyle advice to maintain or improve your health.
Yes! Shoreline offers Telehealth appointments for established patients. Initial visits are not currently available as telehealth appointments.

Yes! Our staff can assist you with the reimbursement process at the time of your visit.
Our new patient appointments are $150 and return visits are $75.
If you have insurance that we do not currently carry our staff can help you get reimbursed through your insurance carrier!
You can book wellness services without using our practice as your primary care provider!
